Terms and Conditions (Abbreviated)

NeuroRehab SLP PLLC provides qualified speech-language pathology services on a private pay basis only.

• Payment is required at the time of service
• We will provide you with a "Superbill" detailing service codes
• You may then submit the superbill directly to your insurance to seek reimbursement
• Please contact your insurance to confirm their terms and reimbursement policy for speech-language pathology services
• We do not communicate with insurance on our clients' behalf and do not verify insurance reimbursement policies

We provide in-home therapy in the Seattle area for maximum convenience, however certain conditions apply, including:

• Distance
• Access
• Pets
• Weapons and other hazards

We will discuss these conditions before beginning services. We reserve the right to discontinue in-home services at any time at our sole discretion.

For added convenience and access, we also extend services via Telehealth throughout the State of Washington. To receive services via Telehealth, please ensure that you have:

• A computer with a camera, microphone, and speaker
• A stable internet connection
• A quiet space for the duration of sessions
• The technical skills (or a care partner's support) to access your email, the internet, a Telehealth platform, and activate your camera, microphone and speaker.

We will walk you through the Telehealth checklist before beginning services. We reserve the right to discontinue Telehealth services at any time at our sole discretion.
